Executive Director/CEO

United Family Medicine is seeking an experienced leader to be its next Executive Director/CEO for our unique Federally Qualified Health Center.

The Chief executive Officer will lead and direct the integration and delivery of high quality comprehensive community-oriented primary care health services for United Family Medicine (UFM) and assure optimum functioning of all aspects of the Center consistent with the mission statement of UFM.

 Qualifications/Experience needed

Demonstrated fundraising success at the individual, foundation, and corporate levels, as well as success at obtaining government grants for health care funding.

Community clinic or FQHC management experience desirable.

CMA/RMA or LPN

Job Description: United Family Medicine has an immediate opening for a Full Time LPN or CMA/RMA (Certified / Registered Medical Assistant) to work days and evenings between 8:00 am and 8:00 pm and occasional Saturdays from 8am to 12pm at our large Family Practice Clinic.  UFM is also a teaching site for the United Family Medicine Residency Program.

Position Details: Permanent Full Time position available. Some Evening hours and Saturdays required. Starting date: A.S.A.P. Salary: Depends on Experience.

Responsibilities:  Rooming patients including taking vital signs, history, and preparing patient and equipment to be seen by the provider. Performs routine procedures including ekgs, audiograms, vision and hearing screenings, ear irrigations, TB skin tests, Spirometry and assisting with minor surgery and other procedures. Administers medications and vaccinations per provider order.

Qualifications:  Candidates should be an LPN preferably three to five years of clinical or related nursing experience or a CMA/RMA, new graduates encouraged to apply.  Prefer ambulatory care experience. American Heart Association BLS certification, RMA/CMA certification. Bilingual candidates encouraged to apply.

Additional Information:  Physical Demands: Standing for long periods of time. Some bending, stooping and stretching. May be required to lift up to 30 lbs. Triage RN

The hours for this position are:    Monday  through Thursday from 8:00 am -5:00 pm

This position will manage all patient triage over the telephone.  Coordinate and collaborate with the Provider Teams, RN’s, Appointment Schedulers to resolve patient calls appropriately based on patient needs.  Provide follow up calls; take messages and document in the Electronic Medical Record as needed.  This could also include tracking paperwork and messages to monitor and maintain workflow and assure acuity needs are met.

LICENSED DENTAL ASSISTANT (FULL TIME)

MISSION

 JOB DESCRIPTION

Perform chair-side assisting of dentist, clinical support services and assigned related work under the direct supervision of the Dental Director and Dental Manager.

 Essential Functions:

  • Assist dentist chair side during patient procedures within the scope of general dentistry using four handed delivery techniques.
  • Proficient in taking clinically diagnostic digital X-rays, and maintain equipment as outlined in the safety and radiation manual.
  • Sterilize, disinfect and maintain instruments and operations according to OSHA and clinic protocols.
  • Perform daily start-up and close-down procedures in operatory rooms and lab.
  • Set-up and clean-up operatory rooms, materials, and equipment needed for dental procedures
  • Take patient vitals and record in chart.
  • Accurately chart results of oral exam as instructed by provider.
  • Perform child prophylaxis, fluoride treatment and place sealants.
  • Take alginate impressions, pour and trim models and prepare cases for outside lab.
  • Monitor nitrous oxide administered by provider.
  • Perform oral hygiene instructions.
  • Handle and dispose of infectious and hazardous waste according to clinic protocols.
  • Answer phone, screen calls, confirm appointments and do recall as needed.
  • Order supplies, perform inventory and inventory audits as scheduled.
  • Attend staff meetings and contribute to planning and workflow of dental clinic.
  • Perform other duties within the scope of the position as assigned by the Dental Director.

 Competency and skills:

  • Good interpersonal and communication skills.
  • Attention to details and multitasking skills.
  • Proficient in four handed dentistry.
  • Understand and adhere to infection control protocol.
  • Good skills and understanding in digital x-ray exposure techniques.
  • Dentix/Dexis is a plus

  Certificates, Licenses, Registrations:

  • Maintain a current active registration in dental assisting in the state of Minnesota.
  • Current in CPR for providers.

Behavioral Health Consultant

United Family Medicine is seeking a full time Behavioral Health Consultant. United Family Medicine is a Federally-Qualified Health Center in St. Paul, where patients receive community-based Family Practice medical care in addition to mental health, social work and other integrated services. Our Behavioral Health team works together in a fast-paced environment and we serve a diverse population. We are looking for an experienced Behavioral Health Consultant who wants to be part of our team.

We are looking for a Master’s level Licensed Mental Health professional who will see primary care patients in an integrated behavioral health context, providing “in the moment” support and interventions for patients with mental health, substance use, and chronic health care challenges. The BHC will support and collaborate with medical providers in managing patients with complex issues and relay diagnostic impressions, interventions and recommendations to the team.

The BHC will complete diagnostic, chemical health and crisis assessments, as well as treatment plans. The BHC will also be responsible for facilitating group therapy within our Medication Assisted Treatment (MAT) program for those with substance use disorders and work closely with medical providers to monitor and provide counseling interventions to patients who are on Medication Assisted Therapy and have chemical health issues.

Interest or experience in trauma-responsive care and working with substance use populations as well as adults, children, and adolescents is preferred.

The successful candidate will have a minimum of 2 years of experience in working with diverse populations as well as the ability to work effectively as a collaborative multidisciplinary team member in a diverse and fast- paced medical setting.

Experience preferred working in an Integrated Behavioral Health or medical setting; LADC (Licensed Alcohol and Drug Counselor) preferred; working in Electronic Medical Records; and Spanish fluency also preferred.
