Terms and Conditions for Use of Communication Platform

Welcome to United Family Practice Health Center’s Communication Platform (“Platform”). By
using the Platform, you (the “User”) agree to comply with and be bound by the following Terms
and Conditions. These Terms and Conditions govern your use of the Platform to communicate
with employees and access various features provided by the Platform.

Please read these Terms carefully before using the Platform. If you do not agree to these Terms
and Conditions, you must not use the Platform.

1. Acceptable Use

1.1 Authorized Use
The Platform is provided for internal communication between United Family Practice Health
Center and its employees. Users must only use the Platform to communicate for work-related
purposes. Any use outside of the intended scope of the Platform may result in restrictions or
termination of access.

1.2 Prohibited Activities
Users shall not engage in the following activities when using the Platform:
 Sending unsolicited or offensive communications.
